I hereby authorize International Collision Center to disassemble my vehicle for an estimate, to order parts along with all necessary material and begin repairs on my vehicle once responsible insurance provider has an approved estimate. I agree that International Collision Center is not responsible for loss or damages to the vehicle or articles left in the vehicle, in case of fire, theft and any other causes beyond our controls, or any delays caused by unavailability of parts, delays in parts shipments by the supplier or the transporter. I hereby grant you and/or your employees permission to operate the vehicle herein described on streets, highways or elsewhere for the purpose of testing and/or inspection.

All insurance payments made directly to you must be paid, IN FULL, to International Collision Center before we can release your repaired vehicle back to you. This includes EFT, Direct Deposits, and Checks for repairs and/or deductibles.

Customer’s Rights: I understand that: (1) I may request a written estimate for repairs which cost more than $25 and may not be charged any amount 10% in excess of the written estimate without my consent; (2) I may request return of any replaced parts except when parts are required to be returned to the manufacturer under a warranty agreement; and (3) I won’t be charged for repairs not originally authorized without my consent. I further understand that while my vehicle is on International Collision Center’s premises, International Collision Center may not be responsible for damage to the vehicle under certain circumstances, and that I should ask a International Collision Center representative about the extent of its responsibility, including the extent of International Collision Center’s insurance coverage.

Direction of Pay: Certain, if not all, repairs may be subject of a claim made to insurance company(s). I/We authorize the insurance company named above to pay the body shop in the full amount for any and all estimate/supplemental payment(s) pertaining to claim(s) referenced above. The payment(s) are to be made directly to the and without any encumbrance to International Collision Center. I understand that any payments received by me from any insurance company concerning the incident that lead to the damage of the vehicle be immediately turned over to International Collision Center (except payments specifically naming personal injury on the check).

Limited Power of Attorney: I hereby authorize International Collision Center or it’s authorized agents to accept on my behalf any and all checks, drafts, or bills of exchange, and to endorse all such checks, drafts, and bills of exchange for deposit into the business account of International Collision Center for credit on my account for the repairs on this vehicle.

Unforeseen Delays: I understand that due to the worldwide supply chain delays, my vehicle parts may be on backorder and may cause the repairs to my vehicle to be delayed and my rental coverage may be exhausted. In that case, the shop will not be responsible for any additional delays to completion or any rental car fees.

Credit card payments above $500 assessed a 3% transaction fee.
